They are only patients when they are with us. While Teachback is the education standard for adherence management coaching, much of what you 'teach' them stays in their room. Did you discuss the 'punishing' aspects of their care plan with them in a way that boosts engagement and ultimately improves health outcomes?
Effective adherence management coaching encompasses: 1. Personalized coaching: One-on-one sessions specifically tailored to meet the individual's unique needs, goals, and circumstances, enhancing patient engagement. 2. Goal-setting and planning: Collaborative efforts in goal-setting and planning help identify and overcome barriers to adherence, ultimately improving health outcomes. 3. Education and support: Offering education, resources, and ongoing support empowers individuals to manage their health effectively. 4. Monitoring and feedback: Regular monitoring and feedback allow for tracking progress, identifying challenges, and adjusting the coaching plan as necessary to ensure better adherence and health outcomes.
Adherence management coaching can bring about numerous benefits, including: 1. Improved health outcomes: By enhancing adherence to treatment plans and medications through effective coaching, patients can achieve better health outcomes, reduce complications, and enjoy an improved quality of life. 2. Increased patient engagement: Adherence management coaching empowers individuals to manage their health actively, leading to heightened patient engagement, increased motivation, and greater self-efficacy. 3. Reduced healthcare costs: Improved adherence, facilitated by comprehensive coaching, can significantly lower healthcare costs, decrease hospitalizations, and reduce the utilization of healthcare resources.
Adherence management coaching can significantly enhance health outcomes for individuals managing a variety of health conditions, including: 1. Chronic diseases: Conditions such as diabetes, hypertension, asthma, and chronic obstructive pulmonary disease (COPD). 2. Mental health conditions: Such as depression, anxiety, and bipolar disorder. 3. Complex medical regimens: Individuals with complex medical regimens, including those with HIV/AIDS or transplant patients, can also see improved patient engagement through adherence management coaching.
Equip your care team to listen, understand each patient’s experience, and address barriers to care. Our behavior-informed training supports compassionate conversations and shared decisions, promoting patient engagement. This approach improves health outcomes through adherence-management coaching and practical steps toward stronger advocacy.

All human behavior rests on a single word: consequences. Applied behavior focuses on what people do. Four of the five consequence categories will assure us of non-adherence. When it comes to following care plans, the medical team desires more adherence and less non-adherence. However, patients often weigh a lifetime of habits against the discouraging aspects of following their care plan.
Let us show you how to identify at-risk patients upon admission, so you can evaluate your care plan's impact on their behavior before they head home, where 8.5 out of 10 may choose non-compliance. We can help you identify the punishers and penalties in your care plans. Through adherence management coaching, I believe we can improve adherence and ultimately improve health outcomes.
Are you open to trying a new approach? It’s evidence-based and significantly more effective than relying on hope, and it fosters greater patient engagement.
Patients often stop taking medications due to various consequences. To help understand why many patients struggle with adherence, we have developed a three-word mnemonic (ICE IF SAD). Care plans can be Inconvenient, Confusing, and Expensive (ICE), which contributes to nonadherence. Additionally, the Illness itself may have no symptoms, leading to the thought, "If I don't feel bad, why must I take these pills?" Forgetfulness acts as the "get out of jail free" card for some. If organic brain disease is ruled out, we must consider other factors (IF). Side effects often lead to nonadherence, and patients may accept the consequences of their disease, requiring better engagement and education to manage their health outcomes effectively. Distrust is another factor; patients have unlimited access to the internet and countless conflicting articles, complicating their understanding of how to manage their condition. This is where nurses are crucial, as they are ideally positioned to provide coaching on adherence and address patients' concerns (SAD).
Our medication adherence training programs focus on improving health outcomes through effective coaching on adherence management. Who is at risk? Why do patients not follow their care plan or continue with life-saving or quality-of-life extensions? Adherence is particularly bleak across all areas of health care delivery. While every patient may appear to adhere to their care plan on the day of discharge, our consultants can assess known behavioral risk factors and evaluate physical deficits that may contribute to non-adherence. We conduct a Consequence Analysis from the patient’s perspective to identify potential pitfalls in follow-up care. This allows us to meet with providers and suggest where patient engagement may falter. We will never alter your care plans; instead, we will highlight where patients are likely to abandon them.

Our adherence management coaching, backed by experienced patient assessment and management coaches, can help you address patient education challenges and boost engagement, ultimately improving health outcomes.

A patient has a 50/50 chance of becoming non-adherent to their care plan within 30 days of discharge. The critical question is: which patients are at risk? To address this, we developed an evidence-based matrix that nurses can implement upon admission to help identify patients who may need additional support in patient education programs. The Wright Patient Adherence Assessment Scale uses a Green, Yellow, Orange, and Red format, giving nurses a quick risk-assessment tool.
Patients in the Green Zone typically have a good prognosis for adhering to their discharge plan and can benefit from a 'Teachback' session with their nurse to enhance patient engagement.
Patients in the Yellow and Orange zones, particularly those with higher scores in these categories, should undergo further evaluation and be guided through the Adherence Management Coaching program to improve their health outcomes.

When a patient may be at risk of not following a care plan, a focused assessment can help identify physical and practical barriers. This evidence-informed tool helps care teams rate risk factors and choose thoughtful, individualized support—building patient engagement and supporting better health outcomes.

Our clinicians bring experience in nursing, nutrition, behavior, and related health concerns. We help care teams look beyond a diagnosis to understand each patient's circumstances and perspective. This whole-person approach supports more coordinated, compassionate care and helps professionals address the practical and behavioral factors that shape engagement and follow-through.

A care plan works best when it fits a patient's everyday life. Our practical training helps healthcare professionals understand how routines, circumstances, and patient perspectives shape follow-through. Learn behavior-informed strategies to strengthen engagement, support sustainable habits, and document patient education clearly and thoughtfully.

Adherence is shaped by a patient's circumstances, barriers, and experience of care. Our practical workshop helps care teams recognize risks, understand the patient perspective, and develop supportive strategies that make follow-through more achievable. Build skills to encourage sustainable habits from the first visit and beyond. Contact us to discuss training for your team.

Every care plan has real-life effects. Consequence analysis helps care teams consider how patients may experience its demands, benefits, and potential barriers. By understanding the patient's perspective, professionals can identify ways to make care more practical and supportive—and help patients engage with a plan that fits their lives.
